Registration Form 2007-2008

Please fill out the form below to register for Pacific Gestalt Institute's training programs. After processing your application, we will contact you if the program you are applying for requires more information.

Registration requires a $500 check deposit ($40 payment for the Short Program). Any cancellation notice must be received one week in advance of the program starting date and an administration fee of $75 will apply. There are no refunds after that date or for missed weekends during the training year.
